  • What insurance providers do you accept?
Excellus Fidelis CDPHP MVP BSNENY CVPH EAP United Healthcare/OPTUM Aetna Cigna Medicaid Empire NYSHIP Anthem North Country Center for Independence
  • Do you offer appointments via telehealth?
Telehealth by SimplePractice is the technology service we will use to conduct telehealth video conferencing appointments. It is simple to use and there are no passwords required to log in. Please note that Face-to-face sessions are highly preferable to telehealth sessions. However, if you are out of town, sick, there is inclement weather, childcare issues or in need of additional support, telehealth sessions are available. If a true emergency arises, please call 911 or any local emergency room.
  • What is your confidentiality policy for minors?
If you are a minor, your parents may be legally entitled to some information about your therapy. Clinicians will discuss with you and your parents what information is appropriate for them to receive and which issues are more appropriately kept confidential.
  • How can I contact the office?
If you need to contact the practice between sessions, please leave a voicemail or text message at (518) 565-9577. We will attempt to return your call within 24 hours.
  • What is your cancellation policy?
Peaceful Pathways Mental Health Counseling Services PLLC is committed to providing exceptional, quality care; however, this is difficult without consistent follow-up visits. No shows and late cancellations are costly to the practice and limit access to care for other clients. It is the clients’ responsibility to notify Peaceful Pathways Mental Health Counseling Services PLLC of a cancellation at least 24 hours (1 day) in advance of the scheduled appointment to avoid the no show /late cancellation fee. Appointments cancelled less than 24 hours in advance are considered a no show and will be charged the $75.00 fee. The 24-hour policy is for business hours. An appointment that is scheduled on a Monday should be cancelled on a Friday by 5pm. We are closed on the weekend. If you are more than 15 minutes late for your appointment, we can work to get you seen in the next available time slot and/or you may be charged a missed appointment fee. Reminders are sent as a COURTESY, but it is the clients’ responsibility for missed appointments whether a reminder is sent. · No shows are calculated based on a consecutive 12-month period · All no-show missed appointments and late cancellation appointments are subject to a $75.00 fee. EMAIL ( info@peacefulpathwaysmhc.com ) or TEXT (518)565-9577 are the BEST ways to cancel an appointment as these options are regularly monitored throughout the day. You can also call and leave a voicemail at (518)565-9577. NOTE: Emergencies arise from time to time in which a late cancellation cannot be avoided. Emergency situations will be evaluated on a case-by-case basis at the discretion of the Office Manager.
  • What is your client termination policy?
Ending relationships can be difficult. Therefore, it is important to have a termination process to achieve some closure. The appropriate length of the termination depends on the length and intensity of the treatment. Clinicians may terminate treatment after appropriate discussion with you and a termination process if we determine that the psychotherapy is not being effectively used or if you are in default on payment. Peaceful Pathways Mental Health Counseling Services PLLC will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose of terminating. If therapy is terminated for any reason or you request another therapist, We will provide you with a list of qualified psychotherapists to treat you. You may also choose someone on your own or from another referral source. Should you fail to schedule an appointment for four (4) consecutive weeks, unless other arrangements have been made in advance, for legal and ethical reasons, Peaceful Pathways Mental Health Counseling Services PLLC must consider the professional relationship discontinued.
  • What is your social media and telecommunication policy?
Due to the importance of your confidentiality and the importance of minimizing dual relationships, Clinicians do not accept friend or contact requests from current or former clients on any social networking site (Facebook, LinkedIn, etc). We believe that adding clients as friends or contacts on these sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our therapeutic relationship.
  • How do I complete Intake Forms?
Electronic Health Records (EHRs) have significantly impacted the healthcare industry, offering numerous advantages such as improved client care, increased efficiency, enhanced collaboration, reduced errors, increased client engagement and increased Security/HIPPA Compliance. Peaceful Pathways Mental Health Counseling Services is a Paperless practice. All intake documentation is completed and reviewed through access to your individualized client portal. In the client portal, you can schedule and reschedule appointments as well as review practice policies and complete intake documentation.
